Refer Patients to Asthma Australia

Keep your asthma patients on track between visits with our free asthma support.

For free asthma support and information, you can refer your patients to our Asthma Educator service. Our Asthma Educators help your patients with asthma, by providing tailored evidence-based support to suit their needs.

Some of the information discussed may include correct use of medicines and devices, asthma diagnosis, asthma triggers, written Asthma Action Plans, Asthma First Aid and much more. Your patient may also choose to opt in for a follow up call if they would like more support.

What happens when you refer

• Patient will immediately receive a text or email to let them know you have referred them to Asthma Australia, and to expect a call
• We will call to book in a telephone appointment with an Asthma Educator
• An Asthma Educator will call the patient and explore their asthma management and make recommendations to discuss with you if anything is not on target.
• Your patient with receive a post call summary email outlining everything discussed with the Asthma Educator and potentially some supporting asthma resources
• If you opted in to receive a patient summary report, it will be sent to the email address you provided, once call has been made

Why doctors like our summary report

The patient summary report provides you and your patient with a record of the information discussed and suggestions made. This can easily be saved to your patient’s profile or MyHealthRecord.

Next time you see your patient you can review the suggestions together and decide which to put in place.

Who can refer

Any health professional can refer to Asthma Australia. This includes general practitioners, pharmacists, nurses, allied health professionals etc.

Who is eligible

You can refer any asthma patients over three years old to Asthma Australia.

We especially encourage referrals for patients you who have multimorbidity or for whom you are implementing a treatable traits approach or multidisciplinary care.
